Scenes Hoisting operation beside the reservoir
On the reservoir site, the iron plate (6000 × 150 × 20mm) on the dam must be removed. It is ready to be hoisted with an excavator and loaded onto the rear trailer.
Accident
When the excavator hoisted the iron plate for the right rotation, the crawler on the left side lifted, and the entire excavator fell to the reservoir, and the operator died.
Avoid
(1) Use reasonable hook device for excavator.
(2) The operator must have corresponding professional qualifications.

Scenes Hoisting pipes into the trench
Use the hook at the excavator bucket to hang the pipeline (about 1.2t) under the trench (depth 4.7 meters). I want to bury the pipeline.
Accident
On the way down, the pipe hit the crossbeam of the trench. The cable was slowly detached from the hook, and the pipe fell and directly hit the workers, causing death.
Avoid
(1) Do not use for any purpose.
(2) Check the hook before starting the operation.
(3) The anti-falling device of the hook is always provided.
(4) When working, do not stay under the hanging object.

Scenes When entering the cab
The excavator's driver left the cab with the engine on, and then placed the clamps near the seat pedals to climb onto the cab.
Accident
When the clamp was put into the cab, the clamp hit the left walking pedal, and the excavator drove left and walked. The driver was crushed by the crawler and died.
Avoid
(1) When leaving the cab, place the bucket on the ground, stop the engine, and remove the key.
(2) Do not bring unnecessary things into the cab.

Scenes When the excavator gets off
The ground at the construction site was uneven and the excavator was about to be unloaded from the trailer. From the ground to the trailer platform, two aluminum ramps were placed.
Accident
The swash plate suddenly broke off, the excavator fell over, and the driver was pressed underneath.
Avoid
(1) When unloading heavy machinery, it should be performed on a flat and solid site.
(2) Use a swash plate suitable for handling vehicles or construction machinery, and set the angle of the swash plate below 15 degrees.

A road construction site with a width of 4M was set up on the mountain side as a wall.
Accident
When a worker was about to pass by the excavator, the excavator's operator did not notice the situation here and performed a turning operation. The worker was squeezed by the rock wall on the side of the mountain and the weight of the excavator and died.
Avoid
(1) Strictly implement the safety instruction of "No persons are allowed in the machine's turning range".
(2) Set up a fence that is prohibited from entering.
(3) Specify the operational command personnel.

Scenes Assist the excavator to carry the water pipe
At the buried water pipe construction site, the excavator lifts the water pipe from the middle of the water pipe (weight 53 kg) with an iron rope and moves it. To prevent the water pipe from shaking, the staff held the water pipe with their hands.
Accident
In order to avoid the stones on the road ahead, the driver urgently changed the walking direction of the hydraulic excavator. The suspended water pipe caused a large shake, and the staff who helped the water pipe fell to the ground and was squeezed by the excavator and died.
Avoid
(1) Implement safety measures such as removing obstacles on the road of the excavator in advance.
(2) Do not perform emergency operations on the excavator, etc. and thoroughly educate the operator.
(3) It is not possible to hold the water pipe directly by hand, and it is necessary to maintain a safe distance by means of tools such as ropes.
